You are sitting on the edge of the exam table, your cat purring loudly against your chest, when the vet leans in with a stethoscope. The needle drops: a loud, rhythmic whooshing sound that fills the quiet room. The vet’s face changes. They use a clinical word that makes your stomach drop, and suddenly the purring cat in your lap feels like a ticking time bomb. You leave the clinic clutching a referral to a cardiologist, bracing for the worst, only to find out the heart is perfectly healthy. This happens more often than anyone wants to admit, and it starts with a misunderstanding of how sound travels through a living, breathing animal.
A cat heart murmur is not a diagnosis. It is a symptom, a sound created by turbulent blood flow. When blood moves smoothly through the heart’s chambers and valves, it is silent. When the flow becomes chaotic, it creates a sound that a stethoscope can pick up. The mistake owners make is assuming that turbulence equals structural damage. It does not. In fact, the most common reason for a loud murmur in a young, otherwise healthy cat is not a heart defect at all, but the sound of blood rushing through healthy, high-volume circulation.
When a cat is stressed, excited, or simply resting after a meal, the heart pumps harder and faster. This increases the volume of blood moving through the valves. The faster the blood moves, the more likely it is to create turbulence, and turbulence creates sound. This is called a flow murmur. It is a normal physical phenomenon, not a disease. A healthy cat can produce a murmur loud enough to mimic the sound of a failing heart, simply because its blood is moving with enough force to create noise. The murmur disappears when the cat relaxes, and the heart structure remains completely intact.
Understanding the difference between a flow murmur and a true structural murmur requires looking at the physics of fluid dynamics. In the cardiovascular system, flow is either laminar or turbulent. Laminar flow is smooth and silent. Turbulent flow is chaotic and noisy. The transition between these two states is determined by the Reynolds number, a dimensionless value that predicts when a fluid will transition from smooth to chaotic flow. In the heart, this transition happens when blood velocity increases, when the blood itself becomes thinner, or when the geometry of the valve opening changes.
Young cats, especially kittens and adolescents, frequently produce flow murmurs. Their hearts are small, and the valves are narrow. When a kitten’s heart rate spikes during a vet visit, the blood is forced through those narrow openings at high velocity. The result is a loud murmur. This is not a defect. It is the physics of a high-output state in a small space. As the cat grows, the heart chambers expand, the valves widen, and the blood flow becomes smoother. The murmur often fades away entirely as the cat matures, leaving behind a healthy heart that was never broken to begin with.
However, not all murmurs are benign. Some are caused by structural abnormalities that require intervention. The most common structural cause in cats is hypertrophic cardiomyopathy, or HCM. In HCM, the heart muscle thickens abnormally, reducing the space inside the chambers and forcing blood to squeeze through tighter gaps. This creates a specific type of turbulence that sounds different from a flow murmur. It is often louder, has a different timing within the heartbeat cycle, and does not disappear when the cat relaxes. Distinguishing between these two requires more than a stethoscope.
This is where echocardiography, an ultrasound of the heart, becomes essential. An echocardiogram does not just listen to the sound; it visualizes the structure. It allows a cardiologist to measure the thickness of the heart walls, check the size of the chambers, and watch the valves in real-time. If the heart structure is normal, the murmur is almost certainly a flow murmur. If the walls are thickened, or if the valves are malformed, the murmur is a sign of disease. The echocardiogram is the only way to know for sure, and it is the tool that prevents unnecessary panic and unnecessary treatment.
Many owners are told their cat has a heart murmur and are immediately prescribed medication. This is a common error. Beta-blockers or calcium channel blockers are used to slow the heart rate and reduce the force of contraction, which can reduce the sound of a flow murmur. But if the murmur is benign, these drugs do not fix anything because there is nothing to fix. They only add side effects and cost to the cat’s life. The goal of treatment is not to silence the murmur. The goal is to treat the underlying disease. If there is no disease, there is no treatment, and the murmur is simply a sound to be ignored.
There is a specific condition that bridges the gap between flow and disease: hyperthyroidism. This is a common endocrine disorder in older cats, caused by a tumor on the thyroid gland that produces excess thyroid hormone. The hormone speeds up the cat’s metabolism, which in turn speeds up the heart. The heart beats faster, pumps harder, and the blood moves with enough force to create a murmur. In these cases, the murmur is a symptom of the thyroid problem, not a heart problem. Treating the hyperthyroidism with medication or radioactive iodine usually resolves the murmur, because the blood flow returns to normal once the hormone levels are balanced.
Another structural issue to watch for is a patent ductus arteriosus, or PDA. This is a congenital defect where a blood vessel that should close after birth remains open. It allows blood to flow between two major arteries in the wrong direction, creating a continuous, machinery-like murmur that is distinct from the systolic whoosh of a flow murmur. PDAs are rare in cats, but they are a classic example of a structural defect that produces a loud sound. The key is that the sound is continuous, present during both the contraction and relaxation phases of the heartbeat, and does not change with stress levels.
Valvular disease is less common in cats than in dogs, but it does exist. Mitral valve regurgitation occurs when the valve between the left atrium and left ventricle does not close tightly, allowing blood to leak backward. This creates a systolic murmur that can be quite loud. Like HCM, this requires an echocardiogram to diagnose. The damage to the valve is structural, and it will not go away when the cat relaxes. The murmur is a direct result of the leak, and it will persist regardless of the cat’s stress level or heart rate.
So, what should you do if your vet hears a murmur? The first step is to wait. If the cat is young, healthy, and acting normally, do not rush to a cardiologist. Give it time. Many flow murmurs fade as the cat grows. If the cat is older, or if there are other symptoms like weight loss, rapid breathing, or lethargy, then an echocardiogram is warranted. Do not treat the sound. Treat the cat. If the cat is happy, eating, and playing, the murmur is likely just noise. If the cat is sick, the murmur is a clue to the underlying illness.
The physics of blood flow is a reminder that sound is not always a sign of danger. Sometimes, it is just the sound of life moving through the body. A healthy heart can be loud. A stressed heart can be loud. A dying heart can be loud. The difference is in the structure, and the structure can only be seen, not heard. An echocardiogram is the bridge between the sound and the truth. Without it, you are guessing. With it, you know.
Next time you hear the word “murmur,” do not panic. Ask for the ultrasound. Ask to see the heart, not just hear it. The sound might be loud, but the structure might be perfect. And if the structure is perfect, the murmur is just a lie told by physics, a harmless echo of a healthy, high-energy life. Your cat is not broken. It is just loud. And that is okay.
